I'm having trouble entering city and state information for my shipping address during checkout. What do I do?
We restrict entering city and state information to reduce the keystroke errors and to ensure that your purchase will be delivered to the correct address. Enter your zip code and once you tab off that field, the city and state will auto-populate. Should there be multiple cities for your zip code a pop-up window will appear, allowing you to choose the correct city from a drop-down menu. Note: Please disable your pop-up window blocker during your order to permit access to all forms and this window.

Why do I need to give you three phone numbers?
We require two phone numbers and the third phone number is optional. This information enables us to contact you if there are problems such as missing forms, delivery or return, or payment plan.

Why do you need my checking account or credit card information?
We only ask for this as a form of backup payment. We securely store your checking account or credit card information and it is not used unless your deductions stop before your purchase is paid in full.
